Tesamorelin + Ipamorelin
Tesamorelin's belly-fat power with a smaller tesa dose, thanks to ipamorelin's pulse. Fasted, before bed; pricey; blood sugar to watch.
What it is
This blend is for the person whose problem is deep belly fat, the hard, round kind under the muscle that diet alone is slow to touch, who wants the one GH peptide with trial data on exactly that. The ipamorelin makes each pulse bigger, so you get the tesamorelin effect at a smaller tesamorelin dose. It is the most expensive GH stack here and the one with the most human evidence.
The user is usually a man over 35 with a waist that has not moved despite a decent diet, or a woman with the same visceral-fat pattern after menopause, and often someone already on TRT. People who want general sleep and recovery and have no belly-fat problem are overspending; CJC-1295 + ipamorelin does that at a fraction of the cost. People with diabetes or prediabetes need to read the blood-sugar paragraph before anything else.
Sleep deepens in the first week, as with any bedtime GH peptide. The belly-fat change is the slow, real one: most see the waist start to come in around weeks six to eight, with the visible change arriving by month three. In tesamorelin's own trials, visceral fat dropped by roughly 15 percent over six months while fat under the skin barely moved, which is a very specific effect: it is not a general fat burner. IGF-1 roughly doubles, which is also why skin and recovery improve. It will not stay off on its own; in the trials visceral fat returned within months of stopping.
Dose the blend by its tesamorelin share. The 16 mg vial is 13 mg tesamorelin plus 3 mg ipamorelin; mixed with 3 ml, a 1.25 mg total draw delivers about 1 mg of tesamorelin and 0.23 mg of ipamorelin, which is the standard. Women start at 0.3 mg total (about 0.25 mg tesa) and cap at 1.25 mg; men run 1.25 mg and can go to 2.5 mg total (about 2 mg tesa). Before bed, fasted, with no food 90 minutes either side. Tesamorelin is fussy: reconstitute with room-temperature water, never cold, and use plenty of it, since concentrated tesamorelin gels. Expect flushing and a sting the first nights. At 1.25 mg a day a 16 mg vial is about 13 days, so a 120-day run is nine or ten vials; budget for it before starting, because quitting at week four wastes every vial so far. Run up to 120 days, then take 2 to 4 months off. That ceiling is our caution, not a sign it stops working (tesamorelin was studied for a year): both halves push IGF-1 and blood sugar, so the 2 to 4 month break lets both settle and is the time to recheck them. Expect some of the belly-fat result to come back during those 2 to 4 months off; that is why you plan the next run. A 10 mg vial is usually 5 + 5, a different ratio, so recheck the math if you switch sizes.
Tesamorelin has an FDA-approved version (Egrifta) for excess belly fat in HIV lipodystrophy, with placebo-controlled trials behind it; ipamorelin has solid human data on GH release. The blend and its use for general body composition have not been trialed, so the belly-fat evidence is real but borrowed from a specific population. Growth hormone raises blood sugar, and tesamorelin's trials showed small rises in glucose and HbA1c in some people; that is the main thing to monitor.
If you want sleep and recovery, CJC-1295 + ipamorelin. If you are over 50 and want a long-term GH effect, low-dose HGH is often cheaper per month and is well tolerated at 1 to 2 IU, though harder to source. If your fat is mostly under the skin rather than deep in the belly, this is the wrong tool; a GLP-1 class drug will do more. And if you have diabetes or sleep apnea, the GH cautions apply in full; a cancer history rules it out.
What people use it for
Belly fat
Most evidenceTesamorelin's placebo-controlled trials in people with HIV-related belly fat cut visceral fat by roughly 15 percent over six months with little change in fat under the skin, and the fat came back after stopping. That is approved-drug evidence, in a specific population. For a general user it predicts a real but targeted effect on the deep belly, not overall leanness.
Growth hormone and IGF-1
Some evidenceTesamorelin roughly doubles IGF-1 in its trials, and ipamorelin reliably adds a clean GH pulse in human studies; a GHRH with a GHRP gives a larger release than either alone. That is why the blend lets you use less tesamorelin. Nobody has trialed the pair together.
Sleep, skin and recovery
Some evidenceDeeper sleep, better skin and faster recovery are consistent user reports and follow from the IGF-1 rise and the bedtime pulse. There is no trial of the blend for these; the evidence is physiology plus experience, and CJC + ipamorelin gets you the same for less.
How it works
- Combines belly-fat focus with a clean GH pulse
- A common stack for body composition
- Tesamorelin has an approved version; the combo itself is not approved
- Worth a doctor conversation, especially around blood sugar
