Retatrutide + Eloralintide

Retatrutide's burn plus eloralintide's fullness in one weekly shot, 4:1. Two levers, one vial, no extra stomach slowing.

What it is

For the person on retatrutide who still eats past full, or who wants the triple agonist and an amylin signal without buying two vials. Retatrutide raises how much you burn; eloralintide tells your brain you have eaten enough, through a separate receptor, with nausea near placebo in its trial. The split is fixed at 4:1, so you dose it by the retatrutide share.

Two groups. People already on retatrutide whose loss has slowed and who notice they still clear the plate: the glucagon arm keeps the burn up, but retatrutide's appetite effect is deliberately weak (about a fifth of semaglutide's by our estimate), and eloralintide supplies the fullness it lacks. And people choosing a first GLP-class protocol who want both levers from day one without juggling two vials, two schedules and two ramps. It is not for someone who wants the food-averse feel of semaglutide; this pair leaves you able to eat, just done sooner.

Weeks one and two are the bumpiest: a few queasy days after each shot, snacking loses its pull, and meals end earlier than you expect. You run a little warmer and your resting pulse ticks up a few beats; that is the retatrutide and it is normal. The scale moves from week three, usually a steady pound or so a week once you are settled. Eloralintide's half-life is about two weeks, so its share takes a month to show fully at each step; judge a dose at week four, not week one. It will not protect muscle: protein and lifting are part of the protocol.

Dose by the retatrutide share. The 12.5 mg vial is 10 mg retatrutide and 2.5 mg eloralintide; mix it with 1.25 ml so 10 units is 1 mg of blend, which is 0.8 mg retatrutide and 0.2 mg eloralintide. New to GLPs: 0.625 mg total a week (about 6 units, 0.5 mg retatrutide) for four weeks. Most settle at 2.5 to 3.75 mg total (2 to 3 mg retatrutide, 0.5 to 0.75 mg eloralintide, under eloralintide's own 1 mg trial start, which is why the stomach stays quiet). Larger people run 7.5 to 10 mg total; 12.5 mg total is the top and is a whole small vial every week. Split the week's dose into two shots about four days apart; retatrutide's six-day half-life rewards it. Step no faster than monthly, because the eloralintide lags. At 3.75 mg a week a 12.5 mg vial lasts about three weeks and the 25 mg vial six to seven. The mistakes: stepping every week because the scale is slow, copying a retatrutide-only dose in total milligrams (you would be taking 25 percent more retatrutide than you think), and adding a separate amylin peptide on top.

Each half has real human data. Retatrutide's phase 2 reported about a quarter of body weight lost at 48 weeks on the top dose, the largest in the class, with a resting heart rate rise as the known cost. Eloralintide's 48-week phase 2 in 263 adults showed 9.5 to 20.1 percent loss with nausea close to placebo at the lower doses. Nobody has trialed the two together; Lilly's own amylin combination work pairs eloralintide with tirzepatide, not retatrutide. So: strong evidence for each lever, a sensible mechanism for the pair (metabolic rate plus central fullness, different receptors), and no data on the sum. Neither is approved.

If you want the proven route, tirzepatide alone is approved and close in effect. If you are new to all of this and want one variable at a time, run retatrutide alone first and add eloralintide later; the blend fixes the ratio, which is convenient but means you cannot raise one without the other. If a heart rhythm problem or uncontrolled blood pressure rules out retatrutide, it rules out this too; eloralintide alone or CagriSema are the amylin routes without the glucagon arm. If you want the amylin lever at a lower price, Retatrutide + Cagrilintide is the same idea at 5:1 with the older amylin peptide and more nausea.

What people use it for

Weight loss (the retatrutide share)

Most evidence

Retatrutide's phase 2 trial in adults with obesity reported average loss of roughly a quarter of body weight at 48 weeks on the highest dose, with no plateau, the biggest result of any incretin drug so far; phase 3 is under way. In this blend it is 80 percent of what you inject and the lever you dose by. The heart rate rise seen in those trials comes with it.

Fullness (the eloralintide share)

Some evidence

Eloralintide's 48-week phase 2 in 263 adults showed 9.5 to 20.1 percent weight loss against 0.4 percent on placebo, with nausea near placebo at the lower doses, because it skips the calcitonin receptor that gives cagrilintide its stomach. At the blend's common dose you get 0.5 to 0.75 mg a week, below the trial's 1 mg start, so expect a gentle fullness signal rather than the trial's full effect.

The two together

Early days

No study has tested retatrutide with eloralintide. Lilly's combination program pairs eloralintide with tirzepatide, where amylin on top of a GLP-1 added loss without adding nausea. The rationale here is the same, metabolic rate from one receptor family and central fullness from another, but treat the pairing as logic, not evidence.

Safety, in brief

Things people sometimes notice

  • Queasiness for a few days after each shot, worst after a step up
  • Feeling done with a meal sooner, and less interest in snacking, which is the effect
  • Running warmer and a resting pulse a few beats higher, from the retatrutide
  • Constipation, or the opposite, while the gut adjusts
