Retatrutide + Cagrilintide
Retatrutide's burn plus cagrilintide's fullness in one weekly shot, 5:1. The plateau-breaker pair pre-mixed at a low amylin dose.
What it is
For the person whose retatrutide loss has stalled and who still finishes every plate: this adds cagrilintide, the amylin peptide Novo pairs with semaglutide as CagriSema, at a fifth of the retatrutide dose. Retatrutide raises how much you burn; cagrilintide tells your brain you have eaten enough, through a different receptor. The 5:1 split is fixed, so you dose it by the retatrutide share.
The classic user is six months into retatrutide, down a good amount, and stuck, with the next step up either not helping or bringing the queasiness back. Cagrilintide is the second lever that experienced GLP users reach for at a plateau, because it works on the amylin receptor instead of the incretin receptors retatrutide already saturates. A second group wants both effects from one vial at the start and does not want to run two ramps. It is not for someone who wants the quiet-stomach version of this idea; cagrilintide also hits the calcitonin receptor, which is where its nausea comes from, and Retatrutide + Eloralintide exists for that person.
Weeks one and two bring a few queasy days after each shot and a noticeable end to grazing; meals finish earlier and eating past full feels bad quickly, which is the cagrilintide. You run warmer and your resting pulse ticks up a few beats; that is the retatrutide. The scale moves from week three, usually a pound or so a week once settled, and cagrilintide's full effect at each step arrives by week four or five. Nausea is the documented cost of cagrilintide, so expect more of it than on retatrutide alone, mainly in the week after a step. Protein and lifting are not optional.
Dose by the retatrutide share. The 12 mg vial is 10 mg retatrutide and 2 mg cagrilintide; mix it with 1.2 ml so 10 units is 1 mg of blend, which is 0.83 mg retatrutide and 0.17 mg cagrilintide. New to GLPs: 0.6 mg total a week (6 units, 0.5 mg retatrutide and 0.1 mg cagrilintide, under cagrilintide's own 0.25 mg start) for two to four weeks. Most settle at 2.4 to 3.6 mg total (2 to 3 mg retatrutide with 0.4 to 0.6 mg cagrilintide, right at cagrilintide's standard 0.5 mg, which is why 5:1 fits). Larger people run 7.2 to 9.6 mg total; 12 mg total is the top (10 mg retatrutide, 2 mg cagrilintide, near cagrilintide's 2.4 mg ceiling) and is a whole small vial every week. Split the week's dose into two shots about four days apart. At 3.6 mg a week a 12 mg vial lasts about three weeks and the 24 mg vial six to seven. The mistakes: stepping up to chase the scale during a queasy week, reading a retatrutide-only dose as total milligrams (you would take 20 percent more retatrutide than you think), and adding a separate amylin or GLP-1 vial on top.
Each half has real human data. Retatrutide's phase 2 reported about a quarter of body weight lost at 48 weeks on the top dose, the largest in the class, with a resting heart rate rise as the known cost. Cagrilintide has placebo-controlled trials on its own and in the CagriSema program with semaglutide, where the pair beat either drug alone by a wide margin and phase 3 is under way. That combination data is the argument for adding amylin to an incretin drug; it was built on semaglutide, not retatrutide, and nobody has trialed this pair. So: strong evidence for each lever, a trial-backed idea, and no data on this exact sum. Neither is approved.
If you want the gentler stomach, Retatrutide + Eloralintide is the same idea with the newer amylin peptide and nausea near placebo in its trial, at a higher price. If you want the pairing that has actually been trialed, CagriSema is cagrilintide with semaglutide. If you are new to GLPs, run retatrutide alone first; a fixed ratio means you cannot raise one lever without the other, and most people never need the amylin lever until a plateau. A heart rhythm problem or uncontrolled blood pressure rules out retatrutide and so rules out this.
What people use it for
Weight loss (the retatrutide share)
Most evidenceRetatrutide's phase 2 trial in adults with obesity reported average loss of roughly a quarter of body weight at 48 weeks on the highest dose, with no plateau, the biggest result of any incretin drug so far; phase 3 is under way. In this blend it is five sixths of what you inject and the lever you dose by. The heart rate rise seen in those trials comes with it.
Amylin on top of an incretin drug (the cagrilintide share)
Some evidenceCagrilintide has placebo-controlled human trials alone and, as CagriSema, with semaglutide, where the combination produced clearly more weight loss than either drug by itself; phase 3 is running. That is the trial-backed case for adding an amylin signal to a GLP-class drug. At the blend's common dose you get 0.4 to 0.6 mg a week, right at cagrilintide's standard solo dose, so expect its full fullness effect and its nausea.
The two together
Early daysNo study has tested retatrutide with cagrilintide; the combination evidence is semaglutide's. The logic transfers, different receptor families, metabolic rate plus central fullness, but retatrutide already raises heart rate and cagrilintide already brings nausea, and nobody has measured what the two do side by side. Treat the pairing as borrowed evidence, not proof.
How people use it
Safety, in brief
Things people sometimes notice
- Queasiness for a few days after each shot, worst after a step up
- Feeling done with a meal sooner, and less interest in snacking, which is the effect
- Running warmer and a resting pulse a few beats higher, from the retatrutide
- Constipation, or the opposite, while the gut adjusts
