What it is

CagriSema is for the person who got real results on semaglutide but stalled, or who found the appetite pull of a GLP-1 alone was not enough. Adding cagrilintide, an amylin peptide, puts a second fullness signal on top. It is Novo Nordisk's late-stage drug, and the gray-market vials are not that product.

Two groups run it. People who plateaued on semaglutide at 1 to 2 mg and want more loss without switching to tirzepatide, and people who want the mental quiet around food that amylin adds (fewer cravings between meals) on top of the slower stomach emptying semaglutide gives. If you have never used a GLP-1, this is not the place to start; the combination is harder on the stomach than semaglutide alone.

Appetite drops in the first week, often within two days of the first shot. Cagrilintide's contribution shows up as feeling done with a meal sooner and not thinking about food afterward. The scale moves from week two or three. In REDEFINE 1, the 68-week phase 3 trial, people lost on average about a fifth of their body weight, clearly more than semaglutide alone over the same period. Expect loss to be fastest in the first four months and to slow as you approach a new set point. It will not spare muscle on its own: lift and eat protein.

Once a week, same day, subQ in the belly or thigh. Start at 250 mcg of each peptide and hold there at least four weeks; step to 500 mcg, then 1 mg, only when nausea has settled. Novo's own titration runs over roughly four months toward a 2.4 mg top dose, and nobody should race it. The common mistakes are stepping up on the calendar rather than on tolerance, and taking the full dose then eating a large meal, which is how you end up vomiting. Keep meals small and slow, and keep fluids up.

This is a real pharmaceutical with large randomized trials, which puts it in a different league from the research peptides on this site. The REDEFINE program showed more weight loss than either component alone. The caveat is REDEFINE-4, the head-to-head against tirzepatide, where CagriSema did not come out ahead. Novo filed with the FDA in December 2025 and it is under review, so it is not on the market, and the vials sold as CagriSema online are compounded or research blends, not Novo's pen.

If you want the strongest proven option, tirzepatide is approved, available and at least as effective. If you are stalled on semaglutide and want to try adding the amylin arm, cagrilintide on its own stacked with your existing semaglutide lets you control each dose separately, which is what most experienced people do. CagriSema as a fixed blend makes sense mainly for simplicity.

What people use it for

Weight loss

Most evidence

In REDEFINE 1, a 68-week phase 3 trial in people with obesity, CagriSema produced more weight loss than semaglutide alone or cagrilintide alone, on average around a fifth of body weight. That is large, controlled human data. For you it means the combination is real, not marketing, and that most of the gain over semaglutide comes from the amylin arm.

Versus tirzepatide

Some evidence

REDEFINE-4 ran CagriSema head-to-head with tirzepatide, and CagriSema did not come out ahead. So it is a strong option but not the strongest, and tirzepatide is already approved. If you are choosing between the two from scratch, that result should settle it.

Blood sugar in type 2 diabetes

Most evidence

REDEFINE 2 tested the combination in people with obesity and type 2 diabetes, with meaningful weight loss and better glucose control. The GLP-1 arm does most of the blood sugar work; amylin adds glucagon suppression after meals. Relevant if you are diabetic, and a reason to have any insulin or sulfonylurea dose reviewed.
