Staying safe
Things people sometimes notice
- Nausea for two or three days after each step up, worse with large or greasy meals
- Constipation that builds over weeks if you do not stay ahead of it with fluids and fiber
- Burping, reflux, and feeling full after a few bites
- Tiredness and feeling cold in the first weeks of fast loss
- Hair shedding a few months in, which is the fast weight loss and settles
- A small bruise or bump where you inject
Stop and get help if (trouble breathing or swelling: call 911)
- Severe, constant belly pain, sometimes spreading to your back; that is a pancreatitis check, today
- Pain under your right ribs, especially after eating, with or without fever; gallstones are more common with fast loss
- Vomiting you cannot keep ahead of, or signs of dehydration: dizziness, dark urine, a racing heart
- A lump in your neck, trouble swallowing, or a hoarse voice that does not clear
- Sudden vision changes if you have diabetes; fast sugar drops can worsen retinopathy
- Hives, swelling of the face or lips, or trouble breathing
Talk to your doctor first, especially if
- You or a family member have had medullary thyroid cancer or MEN 2; this is a boxed-warning contraindication, not a judgment call
- You have had pancreatitis, gallbladder disease, or diabetic retinopathy
- You take insulin or a sulfonylurea; those doses usually need to come down to avoid lows
- You take birth control pills; absorption drops for the first month and after each step up, so use a backup method
- You have surgery scheduled; the slowed stomach matters for anesthesia and most surgeons want it held for a week
- You are pregnant, nursing, or trying to conceive; the label says stop once pregnancy is confirmed and sets no washout, so plan the stop with your prescriber before you start trying
